EasyLiving, Inc. helps transport many clients to doctor’s appointments, medical tests and just about anywhere they need to go.  We know how important good communication is when working together with your medical professionals.  To help you (for yourself or as a caregiver assisting someone else), we wanted to provide a quick checklist you can use to better prepare for a medical appointment.

Before the appointment:

    • Make a checklist of symptoms and concerns.  Document what you are feeling, frequency, when the symptom occurred (occurs), and if you have ever experienced it before.
    • Gather a list of all the medications (including any vitamins/supplements) you are taking including frequency and dosage, or take your medications with you in a bag.
    • Bring (or ensure they are sent) any related records from other visits or tests.
    • Gather your medical history (you may wish to keep a notebook or use an online personal health record)—know any surgeries, diagnoses, close family history, and any pertinent medication history/allergies (i.e. you have tried a certain medication which was ineffective or caused too many side effects).

During the visit:

    • Refer to your checklist of concerns.  Start with your main problem first.
    • Describe any past symptoms & history related to this concern.
    • Ask questions to clarify information you do not understand.
    • Make notes about what was discussed and next steps.

For medicines, tests, and treatments, you may want to ask and note:

    • What’s the name of the medicine (or test/procedure)?
    • Why do I need it?
    • What are the risks?
    • Are there alternatives and what are the pros and cons of each?
    • What happens if I do nothing?
    • For medicines:  How do I take this?  Any side effects I should be aware of?
    • For tests: How do I prepare?

At the end of the visit, ensure you understand:

    • If and when a follow up visit is scheduled, or needs to be.
    • When and how you will receive any test results.
    • Any signs you need to monitor and what you should do if you notice them.
    • All follow up you need to do and details (i.e. when and where you have to go for testing, how to take medications, will you need a ride to a procedure or help afterwards, etc.).
